St. Christopher's Health Record
​Children are required to have a health assessment annually filled out by a physician. We accept the well child form provided by the doctor's office. All forms can be uploaded in the Bright Wheel account. Remember when accessing Bright Wheel forms, to use a browser. Forms cannot be accessed in the Bright Wheel App.  Health and immunization records need to updated to the school any time there is an update with the student.

Authorization for Dispensing Medication Form
Students requiring medication during school hours, such as an Epi Pen or Inhaler. Must have a form from the doctor's office filed in the school office. Medication must be in its original container and labeled with your child’s name and the date medication is left at the school. Medication can only be administered in amounts according to the label directions.
